St. Joe's Mental Health Case Management serves children younger than 18-years-old who have severe emotional disturbances who require case management.
Staff provide the support parents need to make informed decisions about a child’s care.
We use an asset-based approach to build on what works for each child. Mental health case managers are an important part of a child’s therapeutic treatment team. We customize plans to each child and family’s needs.
Mental health case managers support each child’s custom treatment plan by identifying and coordinating the multiple special services the child needs. Case managers can arrange physical or mental health care appointments, extra in-home support, transportation to school and help with transition to or from other services.
