In-home intensive care management for older adults and people living with disabilities in Ramsey and Hennepin counties, individual care plans, advocacy.

Comprehensive in-home assessment to create an individualized care plan, referral and coordination of services in all areas of participant’s life, advocacy on behalf of the program participant, collaboration with other service providers to coordinate and monitor care, reevaluation of service needs on a regular basis to meet changes in participant’s life, online forum and network for caregiver.

Professional care managers assess clients’ needs, develop individual care plans, coordinate services and advocate on clients’ behalf. Volunteers provide transportation to appointments and help older adults with tasks such as grocery shopping.
