Dear Adopted Adult:
We have included links we hope will be helpful in providing information about our Post Adoption services and Social Welfare Society's services. As an adopted adult these options are available to you:
1. You may request copies of documents in your file. This could include information about your birthplace, your foster home or orphanage, and legal documents. We would not know until we review your file what information would be available to you. There is a $40 fee for this service.
2. You may request a search for your birth relatives or foster mother. This request would be forwarded to Social Welfare Society with Catholic Charities acting as intermediary. There is a $100 fee for this service.
3. You may contact the Social Welfare Society directly to request services. Enclosed is a fee schedule for Social Welfare Society Services. We have also enclosed a form that you would send to Social Welfare Society. It includes their address and fax number.
