Caregiver Support
Support, education and assistance for family members and individuals who provide care for adults over the age of 60 or for any adult diagnosed with dementia.


Senior Care Management

Intensive, in-home case management for low-income older adults. Assistance for elders in living as safely and independently as possible, with the goal of delaying or preventing the need for placement in a long-term care facility.
