Reaching out to extremely vulnerable, under-served elders


Intensive case management and advocacy on behalf of vulnerable, under-served elders experiencing homelessness who are facing multiple, complex barriers.

Comprehensive assessment and screenings to determine unmet needs and develop an action plan, connection to resources including Health Care for the Homeless to access care and referrals, assisting clients in moving from shelters to appropriate next step housing, helping clients to enroll in Medical Assistance and other supportive services/resources to assist in maintaining housing stability and successful independent living.

Requirements

Elders 60+ (and those 55+ if Native American), currently homeless in Hennepin and Ramsey counties.
